Position Summary

Teamwork Therapies is seeking a compassionate, motivated, and licensed Occupational Therapist (OT) to provide high-quality rehabilitation services in a Skilled Nursing Facility (SNF). The Occupational Therapist will evaluate residents, develop individualized treatment plans, provide evidence-based interventions, collaborate with interdisciplinary teams, and help residents achieve the highest possible level of independence, safety, and quality of life.

Essential Duties and Responsibilities

Patient Evaluation & Assessment

  • Perform comprehensive occupational therapy evaluations for residents with orthopedic, neurological, cardiopulmonary, and medically complex conditions.
  • Assess functional mobility, activities of daily living (ADLs), instrumental activities of daily living (IADLs), cognition, upper extremity function, visual-perceptual skills, balance, coordination, and safety awareness.
  • Identify adaptive equipment and environmental modification needs.

Treatment Planning

  • Develop individualized treatment plans based on physician orders, resident goals, and interdisciplinary recommendations.
  • Establish measurable short-term and long-term functional goals.
  • Update treatment plans based on resident progress and changing medical status.

Direct Patient Care

Provide evidence-based occupational therapy interventions including:

  • ADL retraining
  • Upper extremity strengthening and ROM
  • Fine motor coordination
  • Balance and postural control
  • Functional mobility training
  • Wheelchair positioning
  • Transfer training
  • Splinting and orthotic management
  • Cognitive rehabilitation
  • Visual-perceptual training
  • Energy conservation techniques
  • Home safety education
  • Fall prevention strategies
  • Adaptive equipment training
  • Pain management techniques

Documentation

  • Complete evaluations, progress notes, daily treatment notes, discharge summaries, and recertifications accurately and within required timelines.
  • Maintain documentation that meets Medicare Part A, Medicare Part B, Managed Care, Medicaid, and payer requirements.
  • Utilize the facility’s electronic medical record (EMR) system.

Interdisciplinary Collaboration

  • Participate in care plan meetings.
  • Collaborate with physicians, nurses, speech-language pathologists, physical therapists, social workers, dietitians, and nursing staff.
  • Communicate resident progress and discharge recommendations.

Patient & Family Education

  • Educate residents and caregivers regarding:
    • Home exercise programs
    • Adaptive equipment
    • Fall prevention
    • Safe transfer techniques
    • Joint protection
    • Energy conservation
    • Home safety recommendations

Compliance

  • Maintain compliance with:
    • Medicare and Medicaid regulations
    • CMS requirements
    • State and federal regulations
    • Facility policies
    • HIPAA requirements
    • Infection prevention procedures

Quality Improvement

  • Participate in quality assurance and performance improvement initiatives.
  • Assist with outcome tracking and clinical quality measures.
  • Maintain productivity standards established by the facility.

Qualifications

Required

  • Graduate of an accredited Occupational Therapy program.
  • Current California Occupational Therapist license in good standing.
  • Current CPR/BLS certification.
  • Excellent written and verbal communication skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.

Preferred

  • Minimum of one year of Skilled Nursing Facility or adult rehabilitation experience.
  • Experience with Medicare Part A, Part B, Managed Care, and PDPM.
  • Experience using Net Health, Rehab Optima, Casamba, PointClickCare, or similar EMR systems.

Knowledge & Skills

  • Strong clinical reasoning and problem-solving abilities.
  • Knowledge of geriatric rehabilitation principles.
  • Familiarity with PDPM reimbursement guidelines.
  • Excellent time management and organizational skills.
  • Ability to prioritize caseloads efficiently.
  • Strong interpersonal and customer service skills.

Physical Requirements

  • Ability to lift, transfer, position, and assist residents safely.
  • Frequent standing, walking, bending, reaching, kneeling, and lifting up to 50 pounds with or without reasonable accommodation.
  • Ability to demonstrate therapeutic activities and exercises.
